CHIEF EXECUTIVE OFFICER

Theresa Matthews, MS, BSN, RN, LCAC, ACRPS

Theresa Matthews is the eldest daughter of the founder, Ann Daugherty. She has been employed at Tara Treatment Center, Inc. since its inception in 1985. Ms. Matthews has held many positions and has been instrumental in the development of each level of care. She received her Bachelors degree in Nursing at Indiana University and her Masters in Business Management at Indiana Wesleyan in Indianapolis, Indiana in 2003. Licensure and Certification: Ms. Matthews is a Licensed Clinical Addictions Counselor and an Advanced Certified Relapse Prevention Specialist (ACRPS) through Terence Gorski’s Relapse Prevention School.

Professional Affiliations: Ms. Matthews is a 1998 graduate of Leadership Johnson County. She was chosen as an Honoree for the 2006 Speaking of Women’s Health. The women honored reflect individuals who have enhanced the quality of life in the community through their efforts in the field of women’s health.

MEDICAL DIRECTOR

James D. Kozarek, M.D.

Dr. Kozarek has been Tara’s Medical Director since June 1996. He is a graduate from the University of Wisconsin Medical School.

Licensure and Certification: Dr. Kozarek is a Board-Certified Family Physician.

Professional Affiliations: Dr. Kozarek is a member of the Indiana Medical Association, American Academy of Family Physicians and the American Board of Family Practice. Dr. Kozarek is affiliated with the Johnson Memorial Hospital, Franklin, Indiana and the Columbus Regional Hospital, Columbus, Indiana.

CLINICAL DIRECTOR

Brian K. Nichelson, PH.D., HSPP, LCAC

Dr. Nichelson has been Tara’s Clinical Director since 1995. He received his Ph.D. from Indiana State University in 1991.

Licensure and Certification: He obtained a Certificate of Proficiency in the treatment of alcohol and psychoactive substance use disorders in December 1997 from the American Psychological Association, and is a Licensed Clinical Addictions Counselor. Dr. Nichelson is endorsed as a Health Service Provider in Psychology and is licensed as a Psychologist by the Board of Examiners in Psychology by the State of Indiana Health Professions Bureau.

Professional Affiliations: Dr. Nichelson is a member of the American Psychological Association, Indiana Psychological Association and the Allied Health Staff at Johnson Memorial Hospital, Franklin, Indiana.

CLINICAL SUPERVISOR

Jennifer Parker, BSW, LAC, ACRPS

Ms. Parker has been employed at Tara Treatment Center, Inc. since 1997.  She obtained her Bachelors in Social Work degree from Indiana University, Indianapolis, Indiana in 1997.  She helped to start the IOP Program for Tara and worked in the Outpatient Program for 10 years before transitioning to the Program Coordinator at our Women’s Residential Facility.  As of 2018, she stepped into the role of Clinical Supervisor.

Licensure & Certification: Ms. Parker is a Licensed Addictions Counselor and an Advanced Certified Relapse Prevention Specialist through Terrance Gorski’s Relapse Prevention School.  She is a certified Team Building instructor.  Ms. Parker has completed the first year of training in Gestalt Therapy at the Indianapolis Gestalt Institute, and is currently enrolled in the Advanced Training Program.

Professional Affiliations: Ms. Parker is involved with Drug Free Johnson County and has been involved in many professional and civic organizations including Johnson County Step Ahead Council.

NURSING SUPERVISOR

Tiffani Waters, RN

Ms. Waters received her nursing degree in 2011 and started her position as a staff nurse.  Shortly after, she stepped into the role of Nursing Supervisor.  She has 15 years’ experience in direct patient care.  She is certified through the National Acupuncture Detoxification Association to provide Acudetox to our clientele.  She has direct operational responsibilities for client care across the continuum.  Ms. Waters is well versed in nursing protocols and standards, policies and procedures, documentation requirements, and The Joint Commission survey process.

WOMEN’S PROGRAM COORDINATOR

Amber Hoff, MSW 

Ms. Hoff has been employed at Tara Treatment Center, Inc. since July 2007.  She previously coordinated Tara’s Outpatient Programs before transitioning into the Women’s Program Counselor position in 2014. As of 2018, she stepped into the role of the Women’s Program Coordinator.

Amber obtained her Bachelor of Arts degree in Psychology from Franklin College, Franklin Indiana. Ms. Hoff received recognition as outstanding Sociology student in 2002. In 2013, Amber received her Master of Social Work (MSW) degree from Indiana University.

Professional Affiliations: Ms. Hoff is a member of the National Association of Social Workers (NASW).

WOMEN’S PROGRAM COUNSELOR

Alexandria Kelly, BA

Ms. Kelly has been employed with Tara Treatment Center, Inc. since January 2018. She obtained her Bachelor’s Degree in Psychology and Certificate in Case Management from Indiana University – Purdue University of Columbus, Indiana in 2016. Ms. Kelly will graduate with her Master’s in Social Work in May of 2018 from the IU School of Social Work in Indianapolis, Indiana. Immediately following, she will pursue her LSW credentials by sitting for the Licensed Social Worker exam over summer 2018. Ms. Kelly has previous experience as a Crisis Counselor for survivors of natural disasters and an ABA Therapist for children on the Autism Spectrum.

Professional Affiliations: Ms. Kelly is a member of the National Association of Social Workers (NASW).

MEN’S PROGRAM COORDINATOR

Barbara Warner, MSW, LCAC, LCSW

Ms. Warner has been employed at Tara Treatment Center, Inc. since April of 2007. She obtained her Bachelor’s degree in Psychology from Indiana University Purdue University Columbus, Indiana in 2003. Ms. Warner received her Master’s in Social Work from Indiana University Purdue University Indianapolis in 2007. Ms. Warner also coordinates student practicum placement on site and enjoys working directly with students, helping to be part of a student’s professional development.   

Licensure and Certification: Ms. Warner is a Licensed Clinical Social Worker, a Licensed Clinical Addictions Counselor and a Certified Team Building Instructor. Ms. Warner has completed the first year of training in Gestalt Therapy at the Indianapolis Gestalt Institute, and is currently enrolled in the Advanced Training Program.

Professional Affiliations: Ms. Warner is a member of the National Association of Social Workers (NASW).

MEN’S PROGRAM COUNSELOR

Ariel Underwood, MS

Ms. Underwood has been employed at Tara Treatment Center, Inc. since January of 2017. She obtained her Bachelor’s Degree in Psychology from St. Mary-of-the-Woods in West Terre Haute, Indiana in 2014. Ms. Underwood received her Master’s degree in Clinical Psychology at the University of Indianapolis in Indianapolis, Indiana in 2016. Ms. Underwood has previous experience in running Intensive Outpatient groups, focused in addictions and has worked with individuals with physical and intellectual disabilities for over eight years.

Professional Affiliations: Ms. Underwood is a member of the American Psychological Association.

SENIOR PROGRAM COUNSELOR

Annie Novotny, BS, LCAC, ACRPS

Ms. Novotny has been employed at Tara Treatment Center, Inc. since 1991. She obtained her Bachelor’s of Science degree in Addictions Counseling from Indiana Wesleyan in Indianapolis, Indiana in 1997.

Licensure and Certification: Ms. Novotny is a Licensed Clinical Addictions Counselor and an Advanced Certified Relapse Prevention Specialist (ACRPS) through Terence Gorski’s Relapse Prevention School. She is also a licensed Adult Probation Officer.

Professional Affiliations: Ms. Novotny is a member of the Indiana Counselors Association on Alcohol and Drug Abuse (ICAADA).

FAMILY PROGRAM COORDINATOR

Tony Brenner, MSW, MSOM, LCSW, LCAC, LAc

Mr. Brenner obtained his Bachelor of Arts degree in Psychology from Purdue University in 1990, his Master of Social Work degree from Indiana University/ Purdue University of Indianapolis in 1992, and his Master of Science degree in Oriental Medicine from the Midwest College of Oriental Medicine in 2005.

Licensure and Certification: Mr. Brenner is a Licensed Clinical Social Worker (LCSW), a Licensed Acupuncturist (LAc), and Licensed Clinical Addictions Counselor (LCAC). He has worked in a variety of settings providing substance abuse and/or mental health treatment services since 1992.

Professional Affiliations: He is a member of the National Association of Social Workers and the Indiana Association of Acupuncture and Oriental Medicine.
