Job Overview:

Coordinates and oversees an assigned area of focus such as the Intensive Outpatient program and/or other organizational divisions

Counsels and aids individuals and families requiring assistance dealing with substance abuse problems, facilitating individual and group psycho-therapy as well as educational groups, and maintaining a client caseload

Must be able to work Day and Evening Shifts

Duties and Responsibilities:

· Assist in maintaining Tara Treatment Center, Inc.’s philosophy, goals, and objectives by applying and adapting these to direct client care

· Establish and maintain therapeutic contact throughout the client’s course of treatment

· Interviews clients, reviews records, and confers with other professionals to evaluate condition of client

· Completes admission process; bio-psycho-social assessments, referral networks, urine drug screens, State, Grant or DCS documents

· Completes insurance pre-certification and re-certification for Outpatient levels of care in conjunction with Utilization Review (UR) staff if applicable

· Conduct timely preauthorization submissions

· Completion of timely concurrent reviews

· Consistent and timely coordination and consultation with the business office

· Professional documentation standards at all times

· Formulates treatment plan collaboratively with client, and implements program for treatment and rehabilitation of client

· Counsels clients individually and in group sessions to assist client in overcoming alcohol and drug dependency

· Counsels family members to assist family in dealing with and providing support for client including family conjoint sessions

· Ability to counsel clients in a Residential/Transitional Residential and/or IOP, OP, or Aftercare

· Refers client to other support services as needed such as medical evaluation and treatment, social and employment services

· Monitors condition of client to evaluate success of therapy, and adapts treatment as needed

· Administers random urine drug screens

· Develop and uphold productive relationships with referral sources i.e. criminal justice system, Department Child Services (DCS), Interventionist(s); maintain updated knowledge of community based programs

· Maintains clear, concise and timely documentation in Sigmund

· Prepare and maintain reports and case histories as indicated/needed

· Ability to conduct chart audits/tracers to assist in review and development of performance Improvement Standards

· Illustrate JCAHO knowledge as applicable to Tara’s organization

· Presents client cases during weekly Multi-Disciplinary Staffing meetings and/or timely presentation of clients in other level of care

· Completes the discharge process including discharge summaries, discharge/transfer forms, breaking down files and routing through appropriate systems

· Attends training on policy, procedures and continuing education

· Maintains an active role as a member of the treatment team and continues to grow in developing skills; serve on committees as deigned

· Planning assigning and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems

· Completes Incident Reports when applicable and route to the Facility Administrator

· Complete work orders and route to Maintenance Supervisor

· Perform any other duties as requested by the CEO and/or Clinical Supervisor

Minimum Skill & Qualifications:

· Bachelor’s degree

· Successful completion of applicable training

· Must have a working knowledge of the disease concept of alcohol/drug dependency, and a thorough understanding of the 12-step fellowships

· Willingness to participate in all aspects of client care

· Active license by the State of Indiana or evidence of working toward the same

· Ability to perform Bachelor and supplementary level groups according to psycho-therapy curriculum

· Possession of a current driver’s license

· CPR certification, must be renewed annually

· Annual Mantoux test or a chest x-ray every two years for tuberculosis
